Bumblefoot treatment?

Spartacus has a problem on one claw, apparently a form of bumblefoot. It started as a small nub on the back of his claw, so he was holding up the claw. The swelling has now grown to the whole area of the joint, and the toes aren't moving now.
We've had him on Baytril for a couple weeks, starting ineffectually via the beak and now by twice daily injections (he just won't take the medicine and splashes his water everywhere).
In summation, it appears to be having little effect going from small nub to swollen joint with no apparent slowing.
Questions:
- is the treatment regime normal? Is the response normal (protracted)?
- do I have to get him off the perches? How do you do that? What does he sit on?
