Can anyone tell me what's going on with this foot? It looks horrible and he is uncomfortable.

First I suggest you soak the foot in chlorhexidine or Hibiclens and using a magnifying glass check to see if anything like fibers are wrapped around the toes. Look like blood flow is constricted. If fibers or even sting or hair are wrapped around loosen and remove.

What does the bottom of the foot look like? What are you using as perches?

Bird may require a vet visit and possible treatment

To me it looks like the bird may have gotten it's foot stuck somewhere, but then got it out but it was damaged. The back toe looks like it's dying. The others have made good suggestions to treat with antibiotic cream. The scaliness could be age related, physical damage, or even scarlet mites. Difficult to say.

I'm inclined to agree with you - my daughter just told me there were night-fright noises in the room last night, even though I keep a blue light on, and his foot was fine yesterday. I washed it and put some antibiotic cream on it, and I'm hoping everything will be ok.

Agree the black toe has lost circulation.... he will likely loose it.
make sure the antibiotic cream does not contain zinc.

usually an ointment is used, so not to glob it on put some of the ointment between your forefinger and thumb and rub then put a thin coat on... Neosporin is fine.

Birds can get stuck in odd spots in the cage... check the gaps between where the edges of the cage meet, had one jump on the side and side down and got a toe hung up where the sides meet...lucky I was in the room. closed the gaps up with zip ties.

another got caught where the grate side on a white breeder cage meets the side of the cage... foot got stuck... the bird would have likely been able to get free if not for the leg band... again I heard the noise of struggling and helped get her free.
